My sister is about 39 and will be starting a 2-year college this fall. Due to her family needs, I can only recommend her to go short cut (rather than spending 4, 5 years at university) by taking a health-related certificate in college.
Any advice on a decent certificate that leads to high-paying job or to start up a small business?

Radiologic Technology is a two year degree, pays good, and is very rewarding.
associates RN is 2 years if you are willing to cram a little more than 12 credits per semester
