All the quality can be destroyed in an afternoon. (A new section leader and test section leader can helpfully modify the system and modify the test system to "reflect the released code". If they are stupid enough to do this, it's a fair guess they will also destroy all evidence of the earlier system that did work.).
This tells me I need to develop a product with "millions of times" the final needed quality, to give me any chance of delivering a usable product.
(The system I deliver is not usable, if it can be later doctored.)
(An unusable product is a very poor quality product, irrespective of the quality of its components or method of manufacture).
