At a farewell morning tea, a former defence honcho unintentionally (it hadn’t occurred to him that this was a terribly unsafe way to develop software) spills the beans.
Relates how a staff member used to alter the code, do a new build, and release the new build for testing at sea.

You’re thinking “The software used at sea was supposed to be reasonably safe to use, how could we trust the change made by a moron”.

As if reading your mind, a visiting head honcho says ”That’s ok, it was a CSO 4 making the changes so they would have known what they were doing”.

Answer:
Sounds like the Peter Principle to me -- People get promoted until they reach their level of incompetence.

Don't worry -- if there had been any real damage or injury or worse, you would have heard about it (and been held responsible).
