Self Funded Insurance Plans?


Question:
My wife requires a procedure which is not covered by my corporation’s self funded health insurance plan. Is there a form I can file with the plans TPA or with my company itself for special cases to appeal for coverage?
You need to work with your insurance company.
You have to work with the HR people in your company.

You may have to pay for the surgery out of pocket yourself, if it is considered an elective surgery.
