The truth, if its not curable don't lead them on. Make sure the patient knows what will occur. Always get a second opinion. As a physician you only have one answer you can give or recommend and that is to fight it.
Its up to the individual to decide on what they will do and the doctor should not get involved unless it is part of the newer laws in some states for Assisted Rights to die. I think that it requires at least two physicians making recommendations, on patients choice.
Its up to the physician if he wants to be involved in these decisions making matters.

I am bound by oath as a physician to do no harm--but if my patient's cancer is incurable and there is uncontrolled and uncontrollable pain or grievous loss in quality of life, I might make sure my patient knew that taking too many of these pain pills, say 15, especially with alcohol, would be fatal, and to be careful with them.
That puts the decision where it needs to be, in my patient's hands.
That's a very difficult question, that no matter what you decide you loose.
