Are most pastors' health insurance plans provided by their church, or must they pay for their own insurance?

It depends on the denomination and the size of the church. All Catholic priests are covered by the Church. No Church of Christ ministers are. Most protestant churches of over 200 in attendance do offer a health insurance plan as part of the pastor's pay package. Most churches under 100 do not. In between churches vary.
My father is a minister. He is on my mother's insurance. She was employed as an elementary public school teacher.
They don't need any health insurance, God takes care of them..
but...i thought preachers didnt get sick because they bathe in the holy water every night...
It depends on each church. We pay for our Pastor-Teachers
insurance and he deserves everything he get. He studies his
guts out for the flock. He is priceless!
I dont know about all churches, but most of the churches I am acquainted with pay for their pastors insurance.
some of each, AND some pay a percentage leaving the pastor to pay the rest.

Some place a cap on the premium they are willing to cover, and the pastor can choose his plan, as long as he knows he's responsible for the remaining portion of the premium.
