Tufts Health Plan Medicare Preferred?

Question: I have Medicare Preferred HMO ID #S00248437 Rx BIN 610415 HMO paying $120.00 per month. I received in the mail a form "HMO Individual Change Form" only. No instructions re my status. You should call the 1-800-medicare number. Or call your regional medicare office. Or call the OIG hotline if you think it is fraud - 1-800-447-8477.
