I found a ProCare in Michigan, and yup, its an HMO owned by physicians. No SEC reports anyhow. NIAC has some info.
Search For Pro Care
Quarterly reports are issued by public companies only (listed in the stock market). I searched for "ProCare" and there is no publicly listed company under that name
not sure for that company
