My father took out a health insurance plan and has lost all paperwork. HELP!?

All we know about this is that it was a Hospital Insurance Plan, address is 13 Baker Street, London. He would now like to make a claim but have no details except the above. Radio Derby did an expose on this which encouraged my father to take out a plan. Any help would be appreciated even if it is just information on how to find out which company it was, as we do have address but nothing else. HELP!

Hi Tracy, You are on the right step. Write to this address you have,and explain the problem to them. They can get into their system,and help track the imformation down for your dad. If you've got an address, just write to them & ask for copies of your paperwork. Better write to them ASAP, or the time limit for making a claim will run out. In fact, you'd be better emailing them, rather than writing, as you'll save a few days where the letter sits in the post. Google the address you've got & see if it comes up with an email address - or even a phone number.
