Career Step and Andrews School are the two best courses, have been around for many, many years. I went to Career Step and have been working at home since I graduated 5+ years ago.
This is not something you can do "off the street." If you get your education from a local college, you most likely will not be hired by national companies that allow you to work at home. The curriculum is just not the same. In plain words -- they don't teach you what you need to know. Private school is the way to go.
For example -- you will learn the words "foramen" and "foraminal." Then you will hear dictator say "foraminal valley." Spell check will never pick that up as wrong. It is wrong. It's foramen ovale.
I believe Career Step course of $800-$1500. There are different levels of enrollment, such as books or online.
You can phone MedQuist. They will confirm that medical transcription at home is legit and tell you the schools that are accredited by them and other national companies.
Spheris is another big national company to work for. They'll tell you, too.

It's a great line of work, very secure, good pay, flexible (although some companies have virtual timeclocks, unless they did away with that realizing what a mistake that is ;)
If you are already transcription literate and just need medical courses, my thoughts would be that you could take a medical terminology course from the local college in your area. Even better, just start sending out your resume by fax or in person to local doctor offices in your area and tell them that as you are getting started you are offering a deal that goes along with your inexperience. I think you could probably get work at the same time you are taking the terminology course and expand from there.
