I believe you can print whatever you want, but if you offer medical services and charge for treatment without being a licensed doctor you'll get in really bit trouble.

No, because MD would mean you are a doctor and you can't put that if you are not. Put MD by your address of your office. Cool, I'm from Maryland too!
Practicing medicine without a license is illegal in all 50 states. If your license was revoked in another state, MD will find out eventually.
