It may be that the picture is not in the right format. I know this does not specifically answer your question, but you may want to try VISTAPrint. They have free cards to get you started and its pretty cheap to upgrade to the premium service. You even upload your own logo. I did this for my real estate gig. They have a ton of templates. I also have gone to Kinko's to print out the cards and they figure out the issue you have with the photo not going on the template for you, as long as you pay for the paper, which was pretty cheap.

MS Word + laser printer and you're done. And a proper pack of paper, of course.
Convert the logo into a format that is accepted by the program and you should be good to go.
